Eligible Requirements: You are eligible to apply for your child if: You live in the city of Worcester; Your child is 3 or 4 years of age (Priority is given to four year olds); You receive Public Assistance; Your child is a foster child; You are homeless; Your child has a documented disability; Your family income falls within the Income Guidelines established by the Federal Government

How to Apply: When Registering your child please bring: Proof of child’s date of birth; Proof of Income; Parent’s picture I.D.; Child’s social security card; Parent’s social security card or passport; Proof that your child has been immunizedand tested for lead and a health insurance card; Proof of residency in Worcester
